Ramoji Group is conducting Vaccination Survey.  Each employee will have to submit their details individually by clicking on the link given below.  Once you click the link, a Form will be opened.

You should feed Employee Code. Ex. 55xxxx; 56xxxx, 57xxxxx.
You should enter your Name
Registered Mobile Number
Gender
Select your Organisation : Margadarsi / Kalanjali
Select your Department : Others
Place of Work: Your Department, Place (Branch / CO / RO)
Select your Age Group:
Have you been tested Covid Positive:
Have you been vaccinated:

All the above fields are mandatory and once you fill all the columns, click the next button and submit.

It is advisable to send the details through their Individual Mobile Phones.
